3. All information must be included in the form. Incomplete entries will not be considered. Your information will be kept confidential and will be discarded at the end of the giveaways.

 

4. Additional requirements may be added
when a different prize is to be
given away.

5. An adult 18 years of age or older
must be available to sign for
the Prize.

at Midnight, December 31, 2001.
Winners will be announced the following Wednesday.
